1. You received a client referral from Clarity’s matching process or elsewhere (colleague, PsychToday, etc)
2. The client did *not* move forward
3. The client no showed or canceled for a consult
4. You never heard back from the client (and it’s been 2 weeks or longer)

Do not use this form for consults who have become regular or recurring clients.

and ensure that we’re able to meet the needs of prospective clients as a group.
